Assistance with Accommodation and Tenancy Obligations
Help to gain a suitable accommodation or maintaining your current home
Your outreach worker can work with you and your trusted persons (informal support networks such as parents, children, extended family, chosen family, or formal support networks such as your clinical manager, legal guardian, Housing ACT, and so on).
What do we do for you with Assistance with Accommodation and Tenancy Obligations?
Everybody has those tasks they just don't have time, energy, or capacity to do around the home. A tap that keeps leaking, a small hole in the wall from a door slamming open, a flyscreen that needs replacing. We can help you to collect quotes from locally trusted trades people and organise those appointments for you. You may wish to replace your furniture and assemble pre-made IKEA furniture - let us be of assistance!
From time to time, you may feel your living space arrangements are no longer safe due to your recent injury or health deterioration. As a result of your injury or change in health status, you are now living with physical disability. You wish to make some modifications at home so you could remain living at your own home. Or you may wish to explore living in a supported accommodation. We will help you navigate.
Send us a message for any task or situation you are unsure about.
What do we do for you as a Support Coordinator
The funding for Support Coordination is usually given for a set amount of hours per week as determined by the Planner, however due to the unique way a Support Coordinator functions the funding must be budgeted accordingly to ensure the needs of the client are met at intake and the signing of the service agreement.
As Support Coordination is unique in its format in that it is a mixture of face-to-face sessions and work done away from the client, the exact way coordination will be billed is agreed upon in the initial service agreement. Generally, there will be a bulk of time left for the plan review process, regular report writing to the NDIA, and the initial setup of the plan. Beyond that, there may be check-ins every fortnight or monthly, changing of services or adding new services in.
