Support Coordination / Case Management

We provide services to clients who are living with mild to chronic disabilities and mental health conditions. 

Our services are fee-for-service, or through the NDIS funding for people who are plan managed or self managed.

We know that sometimes people fall through the gaps. We don’t want people to not have access to support services due to non eligibility to the NDIS, or financial reasons. Please talk to us, we may have a scope to support a small number of people at no cost depending on needs, we have the capacity to link you up with other companies or organisations who may be able to assist through the GP Health Care Plan, Chronic Disease Management Plan, and any other community or government funded avenues.

Coordination of Your Supports

A Support Coordinator helps you achieve your NDIS goals by getting you connected with service providers in the community, as well as resolving points of crisis.

What do we do for you as a Support Coordinator

We will be there to help you see your NDIS goals and recovery goals into fruition. We will work with you to identify how you would like to implement your NDIS plan: the sort of organisations or helpers you would like to engage with, and the sort of activities you would like to participate in. This may involve researching a variety of providers in the community to find the one that best suits your needs, attending NDIS meetings with you, helping you navigate the portal, or helping negotiate terms of service with other providers. 

We will also write regular reports to the NDIA as to how you are achieving your goals and how successful your plan is being implemented.

Another goal of Support Coordination is to ensure that you learn how to coordinate your own plan, so as your needs change and evolve over time you are capable of organising your own supports.  

Typically, the budget for Support Coordination will decrease with each new plan - however in some circumstances constant Support Coordination is required, such as in cases where the client will never be able to manage their own plan full time.

What do we do for you as a Support Coordinator

The funding for Support Coordination is usually given for a set amount of hours per week as determined by the Planner, however due to the unique way a Support Coordinator functions the funding must be budgeted accordingly to ensure the needs of the client are met at intake and the signing of the service agreement.

As Support Coordination is unique in its format in that it is a mixture of face-to-face sessions and work done away from the client, the exact way coordination will be billed is agreed upon in the initial service agreement.  Generally, there will be a bulk of time left for the plan review process, regular report writing to the NDIA, and the initial setup of the plan.  Beyond that, there may be check-ins every fortnight or monthly, changing of services or adding new services in.
